Use of FCPS facilities

Please complete an opt-out form only if you want FCPS to omit or exclude your child. Otherwise, your permission is assumed. 

Example: If a Media Coverage Opt Out form is on file at school, FCPS will not use your child's name or photo in website news and social media posts. 

Digital Submissions

• Media Opt Out -- the student or their work appearing publicly in photographs, articles, audio/video broadcasts, and/or interviews
• FERPA Directory Information Opt-Out, with Yearbook Exclusion (Please use the paper form below.)
• Military Recruiter Opt Out -- release of information to military recruiters (high school only)

FCPS prefers you to use the electronic forms above, which are more efficient.

However, you may download and print the paper form below if you prefer.
