Your child's school will let you know by email which forms you need to complete and turn it for this school year.

Request for extension of parent notes (medical absence) | Note: This extension form is only for students with a chronic, ongoing illness.

Only submit these forms if you want FCPS to exclude or omit your child. Otherwise, your permission is assumed.

Media Opt Out -- the student or their work appearing publicly in photographs, articles, audio/video broadcasts, and/or interviews
FERPA Directory Information Opt Out -- release of the student’s directory information to persons and organizations outside of FCPS
FERPA Opt Out with Yearbook Exclusion
Military Recruiter Opt Out -- release of information to military recruiters (high school only)
