Digital Forms for Families

At the start of each academic year, schools need to collect several forms from their families. Your child's school will let you know via email which ones to submit. To streamline this process, you can complete the AUP and Chromebook User forms electronically since only the parent/guardian signature is required. These forms are available in several languages.

Please use your child's Student Number to access the form.  If you do not know it, call the Technology Help Desk at (859) 422-5555.

• Acceptable Use Policy (AUP) - required
• 1:1 Chromebook User Agreement - required

• Request for extension of parent notes / English (medical absence) | Spanish / Request for extension of parent notes
• Intent to Home-School 

The opt-out forms below are not required. Please submit an opt-out form only if you want FCPS to omit or exclude your child. Example: media coverage (website news, photos, social media posts, external media like local TV, etc.)
