Employee Vaccination Incentive Program
All full- and part-time FCPS employees who are fully vaccinated against COVID-19 by Dec. 1, 2021, are eligible for a $200 stipend if they submit documentation of the vaccination by the Dec. 3 and Jan. 10 deadlines. Substitutes who have worked at least 10 days during the 2021-22 school year, with one of those days being within a month of the payment, are also eligible.
Have you lost your proof?
The small, white COVID-19 card you received at the time of vaccination was issued by the Centers for Disease Control & Prevention. If lost or destroyed, it cannot be replaced with another CDC card. However, you can provide a different form of proof for Fayette County Public Schools in order to receive your $200 stipend.
- If you received the vaccine through the Lexington-Fayette County Health Department, simply request your immunization record by emailing the completed form to LFCHD. For questions, contact the county's COVID-19 Call Center at (859) 899-2222.
- If you got the shots from another source, the provider can generate a COVID-19 report through the Kentucky Immunization Registry. You can also obtain it yourself through the state registry.
