Seizures (epilepsy)

  • If your child has a history of seizures or epilepsy, please complete these forms so the school nurse or appropriately trained school personnel can perform or administer specific medical treatment and procedures. Details from the parent/guardian help us work with your student to minimize unnecessary restrictions, feelings of being treated differently, and possible absenteeism.

    Also, please send a current picture of your student so he can be easily identified. This information will be distributed to school personnel on a need-to-know basis and might include bus drivers, substitute teachers, cafeteria staff and others who work with your student daily.

    Seizures 101 -- the basics

    Always let us know of any changes in your student's medical condition or emergency daytime phone numbers.

    If daily medication, EpiPens, inhalers, Diastat, or Glucagon are to be taken on a field trip, a trained FCPS staff member must accompany the student if a parent/guardian is not available to attend. Daily medication and emergency medication administration trainings are offered at the beginning of the school year and are good for one academic year.
