If your child has diabetes, please complete the necessary forms to enable us to best accommodate the student's needs at school. This information is necessary for the school nurse or appropriately trained school personnel to handle specific medical treatment or procedures. Details from the parent/guardian will help us work with your student to minimize unnecessary restrictions, feelings of being treated differently, and possible absenteeism.
Along with these completed forms, please send a current picture of your student so he can be easily identified. This information will be distributed to appropriate school personnel on a need-to-know basis and may include bus drivers, substitute teachers, cafeteria staff, and others who work with your child daily.
Always let the school know of any changes in your student's medical condition or emergency phone numbers.
If daily medication, EpiPens, inhalers, Diastat or Glucagon are to be taken on a field trip, a trained FCPS staff member must accompany the student if a parent/guardian is not available to attend. Daily medication and emergency medication administration trainings are offered at the beginning of the school year and are good for one academic year.
