Campus Portal:  

This application gives families immediate access to students' attendance records, teacher emails, district announcements, and more. You can use this tool to update phone numbers, emails, and contact names which will immediately be accessible to teachers. You may access Campus Portal and FAQs in both English and Spanish at www.fcps.net/ic. 

Maxwell Transportation/Emergency Contact Form:  

This form will enable you to officially designate your child’s primary method of transportation and provide the school with key contact information for emergencies. Please take a moment to provide the emergency and transportation information by completing the following form http://bit.ly/Maxtransportationform.  

Online Health Forms:  

Through the following link, you are able to access pertinent forms that are required by the Lexington-Fayette County Health Department for each year in school. If your child receives medication at school, please take a moment to access the appropriate forms prior to our first in-person day to assist with our medication drop-off process. https://www.lfchd.org/school-health-nursing-services/ 

Fayette County Public Schools Daily Home Screening for Students  

This checklist should be used at home for you to conduct a quick check of your child’s health each morning before your child leaves for school. If you are not sure if your child should be kept at home from school, this guide should help.
