OCTOBER 17-28, 2005

  • Open enrollment for health insurance, dental, Unum life/AD&D, disability, cancer check and flexible spending accounts

The Kentucky Employees Health Plan will be going self-insured. Self-insured means the Commonwealth will be assuming the risk rather than the carrier. The Commonwealth will pay a Third Party Administrator (TPA) to process health insurance claims and to access its providers. The TPA for 2006 is Humana. Express Scripts, Inc. will administer pharmacy benefits. You will receive one insurance card titled Kentucky State Insurance. The current rates and plans are the same for next year.

Important! Plan year 2006 will NOT be a total re-enrollment as previously announced.

If you have health insurance and wish to keep the same coverage and elections, you do not need to reenroll.

If you wish to make a change to your health insurance, YOU MUST WEB ENROLL, using the Identification Card you received from the state. (Do not use a paper application unless instructed to do so by the FCPS insurance coordinator or assistant coordinators.) (Web Enroll)

If you are presently waiving your health insurance and want to continue waiving for next year you must complete a new cafeteria plan paper form ONLY. (Forms)

If you wish to pretax your own dollars, again or for the first time, and participate in the
medical or dependent care flexible spending accounts, you must renew your annual pledge and complete a new cafeteria plan paper form. (Forms)

If you are presently enrolled in dental/cancer/Unum life/AD&D/ or disability insurance, and wish to keep them, do nothing, they will roll over to next year.

If you wish to drop dental/cancer/Unum life/AD&D and/or disability benefits, you must submit your request for cancellation using the paper cancellation form. (Forms)

IMPORTANT CHANGE FOR 2006: Dependents who will turn 24 anytime between January 1, 2006, and December 31, 2006, are not eligible to be enrolled for 2006.

You should have one of each of the following: personalized employee benefit summary, state health insurance handbook, cafeteria plan election form, this handout, which contains a cancellation form and payroll chart.

Applications for the dental, cancer check, Unum disability and life insurance are available on the Web (forms) and at help sessions. Provider directories available on line kyhealthplan.humana.com or (877) 597-7474. The State Office of Insurance web site is http://personel.ky.gov/stemp/dei/default.htm.

Please take advantage of all the resources available to you: Web sites, health insurance handbook, benefits fair, and help sessions. Do not wait to the last minute to take care of this very important business! You are responsible for keeping copies of everything you submit during open enrollment.
