Post-Secondary Transition Mentor Program

FCPS coordinates this mentor program to assist high school students ages 18-21 with disabilities to transition successfully to post-secondary education and adult life.

The program gives students a chance to attend Eastside or Southside Technical Center with support and prepares them for life after high school by focusing on pertinent life skills and opportunities to further their education.

The goal is to connect these young adults with services and supports within their community that will lead to success after high school.
