Proof of identity & birth date
A legal copy of the child's birth certificate or another official document proving the child's identity and date of birth is required for school registration.
These are acceptable alternatives:
- Valid passport or federal government-issued document
- Prior school records (other than FCPS) identifying student by name and date of birth
- Custody order or parent divorce decree with student’s name and date of birth
- Immunization records
- Valid Kentucky driver’s license or permit
You can get a birth-certificate application from the assigned school or from the Fayette County Health Department, 650 Newtown Pike; call (859) 252-2371.
Immunizations
All students must have a valid Kentucky immunization certificate. Kids are required to be immunized by schedule for DT (diphtheria and tetanus toxoids), DTP (diphtheria, tetanus and pertussis) or DtaP (diphtheria, tetanus and acellular pertussis); OPV (trivalent oral polio virus) or IPV (inactivated polio virus); hepatitis B; Hib (haemophilus influenzae type B conjugate); MMR (measles, mumps, rubella); Td (tetanus and diphtheria); and varicella (chickenpox).
Medical Examination
Every student, including transfers, must have a medical exam within one year of entering school. And all incoming sixth-graders must have another physical within one year of school's start.
Eye Exam
All kids between 3 and 6 entering school for the first time must have an eye examination by an optometrist or ophthalmologist no later than Jan. 1 of the school year.
